Job Purpose:
To provide an urgent referral service and single point of contact for Midlothian Intermediate Care Service and advise clinicians on appropriate pathways or sites
Service Scope:
Patients over the age of 65 within Midlothian
Key Responsibilities:
Referral management, patient tracking, bed allocation, daily reporting, administrative support, data management, confidentiality compliance
