Target Group: People aged over 65 years (over 50 for Aboriginal and Torres Strait Islander People)
Service Context: My Aged Care referrals for Allied Health Services
Service Delivery Setting: Home, CatholicCare offices or hubs, community, individual and group basis
Employment Requirements: Eligibility for AASW membership; Working With Vulnerable People (WWVP) card; National Criminal History Check; Current driver’s license
Key Functions: Service delivery, group work, records management, quality assurance, external relationships
Framework: Person centred, wellness and reablement or restorative framework in line with CHSP Manual
Compliance Standards: Home Care Standards; WHS; EEO; Confidentiality policies
